Aging & In Home Services Case Management

Aging and In-Home Services’ Case Managers are certified professionals adhering to uncompromising Case Management standards. Case Management Services is pivotal and, perhaps, the most important component for helping our clients realize their maximum independence. Case Managers serve as the link between clients and direct service providers.

Our Case Managers:

  • Monitor to assure satisfaction
  • Adjust to meet changing needs
  • Advocate to best represent clients

As the single point of entry for individuals in need of funding assistance for long-term care services, we also have Case Managers completing Pre-Admission Screening for Nursing Facility placement.
